Assessment Procedure and Questionnaire

Please complete the Assessment Form below. Your answers will be treated in confidence. We have procedures in place should a conflict become apparent.

When completed, you can choose to post the Assessment Form, scan it and email to assist@inventwa.com.au or, bring it to our next meeting. 
If you have a prototype, model or drawings, please bring them to your assessment.
